Donation Reminders
  • Let your family members know about your decision to donate.
  • Complete the Consent Form, Authorization for Postmortem Examination (autopsy permit) and Authorization for Release of Medical Information.
  • If you or your loved one is in a retirement home, nursing home or hospice, let the facility know about your decision to donate and give them a copy of the autopsy permit.
  • If funeral prearrangements have been made, give the funeral home a copy of the autopsy permit to keep on file.
  • If death is imminent, or as soon as possible after next of kin or other family member has been notified of the donor’s death, please contact the Brain Bank at 608-256-1901 ext. 11767 (8 a.m. to 5 p.m.) or evenings, weekends or holidays at 608-265-7000, pager number 5332. Calls are taken 24 hours a day, 7 days a week.
